Who should be on my team?

Your diabetes care team may consist of your family doctor, a registered nurse, a registered dietitian and other health care providers such as a social worker, a pharmacist and an endocrinologist. Most importantly, the team should include you and your family.

Your diabetes education team will work with you in a group, in one-on-one sessions, or sometimes both. It depends on what is available where you live. Group education enables you to meet others with similar concerns. You can share your experiences and get answers to questions that you may not have thought to ask yourself.

One-on-one sessions let you work closely with your diabetes education team. Every person living with diabetes is different. It’s a good idea to speak with a diabetes educator individually about setting, measuring, and achieving your short-term and long-term goals.
